We work in collaboration with the men we support to create care plans and goals for the future, including a discharge pathway to a less restrictive setting. Our multidisciplinary team offer specialist interventions, therapies and activities tailored to each individual.
Our model of care is shaped by the people we support. We use a wrap-around approach that integrates carers, family, and community resources to achieve sustained success.
Cygnet Churchill is a service built on recovery principles of hope, empowerment, wellness, personal responsibility, community focus and connectedness. We are committed to continually improving our service user’s experience.
